Request a Medical Leave of Absence

Once the semester begins, an enrolled student interested in medical leave must complete their request by the last day of classes that semester. A student may also take medical leave prior to the beginning of the next semester even if they have enrolled in courses but are unable to start classes. 
Steps for Taking a Medical Leave
  1. Talk with the Office of the Dean of Students: All students requesting to take a Medical Leave of Absence need to have a conversation with the appropriate Dean of Students resource.  
  2. Meet with the Director of Counseling and Wellness or the Director of Health Services, depending on the nature of your leave.  
  3. If you are an International student, talk with your Class Dean about additional implications. 
  4. Complete the Intent to Withdraw form after you have met with your Dean and once you have determined that a Medical Leave is the best course of action. 
  5. Prepare to leave campus: Students residing in campus housing, are required to move out within 48 hours of completion of the Intent to Withdraw form. Contact the Mail Center to provide a forwarding address while away from campus. 

*Once the Intent to Withdraw form is processed, the appropriate departments (i.e. Registrar, Bursar, Office of Residential Life, etc.) will be notified.  

Medical Leave of Absence Letter

Once the MLOA is formalized, the student will receive a letter by email confirming their change of status, outlining next steps regarding their treatment plan, and reviewing the re-enrollment process. The College reserves the right, consistent with applicable state and federal privacy laws, to notify a parent or guardian of their student’s change in status if circumstances warrant and if it is believed to be in the best interest of the student and the College community.

Treatment Plan

Within a timeframe articulated in the MLOA letter (typically two weeks after the leave is formalized), the student is expected to submit a treatment plan developed with their provider(s) to either the Director of Health Services or Director of Counseling and Wellness, depending on the nature of their leave.
